A Biopsy Kit (supplies for 2 biopsies) contains:

• 2 labeled bottles containing 10% neutral-buffered formalin
• 2 biopsy request forms (NCR paper)
• 2 postage paid bubble pack mailers with protective bag

There is no charge for biopsy kits. We will mail you more than one kit if you request below. Every time you send in a biopsy, you can check off on the bottom of the "Request for Tissue Examination" form that you want more kits and we will send them to you.
