The Oral Pathology Department at the Medical College of Virginia, School of Dentistry of Virginia Commonwealth University continuously seeks to provide high quality oral and maxillofacial pathology diagnostic services. To assist in this effort, your assessment of laboratory services is valued and greatly appreciated. Please respond to the following questions and submit your response to us as directed below.

INSTRUCTIONS: Please indicate the degree to which you agree with the following statements as they pertain to the Oral Pathology Diagnostic Service ("the Laboratory") and its personnel.
