The Oral Pathology Department at the Medical College of Virginia, School of Dentistry of Virginia Commonwealth University continuously seeks to provide high quality oral and maxillofacial pathology diagnostic services. To assist in this effort, your assessment of laboratory services is valued and greatly appreciated. Please respond to the following questions and submit your response to us as directed below.

INSTRUCTIONS: Please indicate the degree to which you agree with the following statements as they pertain to the Oral Pathology Diagnostic Service ("the Laboratory") and its personnel. Record your response by entering a number between 1 and 10 in the box after the statement. The lower the number the more you disagree with the statement. In the event that you have no opinion concerning a specific issue, or you feel you don't have sufficient data to answer the question, please note this in the "Comment" section of that question.
