1. Complete the Graduate Program in Periodontics Application for Admission.

2. Completed application should be mailed to:

3. Request the following to be mailed to the same address:

a. At least three (3) letters of recommendation from persons familiar with your personal and professional qualifications for advanced education, including two from faculty members of the Department of Periodontics at the school of your graduation.

b. Complete transcripts of your college and university records.

c. National Board scores (Parts I and II).

A personal interview may be requested by the Director of Graduate Periodontics before final action is taken on the application.  Interviews will be scheduled during August 2008.

Applications will be accepted beginning on June 1, 2008 for the class of 2009.  Applicants who are current undergraduate dental students should submit transcripts which include at least the entire third year courses. To insure full consideration, completed applications should be received by August 10, 2008.
