Insurance and financial policies
Our charges are based on the type of service received during your visit(s). For all uninsured patients, or for non-covered services, we request payment at the time of service. Coverage by your insurance company
Procedures performed by oral and maxillofacial surgeons at the VCU Department of Oral and Maxillofacial Surgery can be covered either by your medical or dental plan, depending on the nature of the procedure. When you call our office for an initial appointment, please verify that we participate with your insurance carrier.
The department participates with various dental and medical HMO and PPO networks, as well as with most indemnity plans, including Anthem Blue Cross/Blue Shield, Key Care/Key Advantage, Healthkeepers, HMO, Champus, MDIPA, Medicaid and Medicare.
We also participate in several other major insurance plans and HMOs. In addition, we participate in dental insurance including all Blue Cross dental products, Delta Dental, Dental Max, Delta USA, Cigna Dental and Met Life Dental.

Also, for your convenience, the department offers several payment plan options. These include, but are not limited to; Visa, MasterCard, Discover Card and American General Finance. We accept Medicaid and Worker’s Compensation, with limitations, after authorization is obtained through the employer and carrier.
Your insurance policy may cover all or part of your bill. The different health plans have varying deductibles and coverage. We will prepare and submit our claims to your insurance company. Please contact your insurance carrier to inquire about any unpaid claims; ultimately it is your responsibility to pay for services rendered.
Health maintenance organizations (HMOs)
If you belong to a health maintenance organization, you
may need to receive prior authorization from them before
seeing one of our doctors. In some cases, you must obtain
a referral form or number, and/or contact your primary
care physician (PCP) to authorize your visit. All referral
forms must be presented to our staff before seeing one
of our doctors. If you do not have the required
authorization and/or referral, the insurance company
will not pay for the services and you may be held responsible
for the entire fee.
Other managed care programs
Your health insurance plan may have a separate managed care company handling your dental health benefits. We request that you provide this information to us prior to your first visit. We will be happy to assist you with any questions you may have about managed care.
Co-payments
Most insurance coverage still requires some co-payment from you; please be prepared to pay these deductibles or co-payments at the time of service. Our staff will assist you in determining your co-payment prior to your initial visit, including contacting your insurance company, if necessary.
If you have an attorney assisting you in obtaining compensation for injuries or damages, we will, with your permission, provide him or her with information about your care. The ultimate responsibility for payment of fees, however, is yours.
